Collecting Intake Documents from Clients

FullSlot lets you request intake documents — forms, insurance cards, consent agreements, or any file — when a client joins your waitlist. Clients upload from their phone or computer without creating an account or logging in.


How to enable document collection

Document collection is enabled per appointment type.

1. Go to Settings in the sidebar 2. Click Edit next to the appointment type you want to configure 3. Check Request intake documents at sign-up 4. Click Save changes

That's it. The next time someone joins that waitlist — via your embed widget, booking page, or QR code — they'll see an optional file upload field.


What the client experience looks like

When a client visits your public waitlist form and document collection is turned on:

1. They fill in their name, phone, and email as usual 2. Below the contact fields, they see: "Attach a document (optional — PDF or image, max 10 MB)" 3. They tap Choose file and pick a file from their phone, camera roll, or computer 4. They submit the form normally

No account, no password, no extra steps.

Supported file types: PDF, JPEG, PNG, WebP, HEIC Maximum file size: 10 MB per upload


Where documents go

Once a client uploads a file, it appears in their profile under Documents.

To view it: 1. Go to Clients in the sidebar 2. Find the client and open their profile 3. Scroll to the Documents section

You can click any file to open it, or delete it if it's no longer needed.


Uploading a document on behalf of a client

If a client sends you a document outside of FullSlot (email, text, paper scan), you can upload it manually from their profile:

1. Open the client's profile from the Clients page 2. In the Documents section, click + Upload 3. Select the file and click Upload


Frequently asked questions

Is the upload field required? No, it's always optional. Clients can join the waitlist without uploading anything.

Can clients upload multiple files? Currently one file per join form submission. Staff can upload additional files manually from the client profile.

Can I collect documents for some appointment types but not others? Yes. The setting is per appointment type, so you can enable it for a service that requires an intake form (e.g. a new client facial) and leave it off for others (e.g. a quick touch-up).

Are files stored securely? Yes. Files are stored privately and are only accessible to you and your team — they are not publicly accessible.
